You allude to the Torah* reading on a fast day. On this subject, you will consult what is said in the addendum at the end of the Reshimos* on Eichah*.
There are several customs regarding the reading of the rebukes (note: found in Parshas* Tavo and also in Parshas* Bechukosai). On this subject, you will consult the Sha'arei Ephraim*, Gate 7, chapter 23. In the synagogue here, the custom is that the reader himself ascends to the Torah* (note: for the reading of these rebukes) without being called by his name, and he does indeed recite the blessings at the beginning and end of this reading.
