According to what is reported, Dr. Shapiro, whom you mention, is a specialist in this matter (pertaining to fertility). He requests a direct examination, after seminal fluid has been collected from the woman following marital relations (that is, semen originating from the man). He considers this necessary to determine the treatment.
My father-in-law, the Rebbe (the Frierdiker Rebbe), gives the following directive regarding this (see letter 2728): "One must practice mesiras nefesh* and undergo this examination."

... הד"ר שפירא שי', שמזכיר אודותו, כפי הנשמע הוא מומחה במקצוע דנדו"ד. ואם הוא דורש בדיקה באופן ישר לאחרי שכבר בדק בלקיחתו אח"ת מן האשה, ואומר שזה נוגע לקביעות דרכי הרפואה, ישנה הוראה למעשה בזה מכ"ק מו"ח אדמו"ר, אשר "מען דארף זיך מסנ"פ זיין און טאן דאס".
