Greetings and blessings,
I am responding to your letter, which was undated.
You know the instruction of our holy masters that one must follow the prescriptions of doctors. Now, the same applies to you, but you seem to do so only intermittently, and during certain periods you cease to listen to them and depart from their prescriptions.
The revealed dimension of the Torah* and Chassidus* establish the value of the Jewish body. It must therefore be in good health and whole. It is in this way that one can serve Hashem without difficulty, and moreover, the Baal Shem Tov* (see letter no. 3381) applies to this body the words of the verse: "You shall surely help him."
In our era, conduct contrary to this (that of fasts and mortifications) often stems from a desire for ostentation, or even from what is worse than that. Therefore, abandon this way of acting! In the future, at least, you will ask the doctor for detailed instructions regarding what you should drink and eat. It will surely be possible to ensure that these do not contradict the Shulchan Aruch*, Heaven forbid. You will then follow them and, as a result, you will heal very quickly and regain your health.

במענה על מכתבו - בלי הוראת זמן הכתיבה -.
הנה ידועות הוראות כ"ק נשיאנו הק', אשר צריך למלאות הוראת הרופאם מומחים, וה"ה בנוגע אליו שכנראה ממלא הוראותיהם לסרוגין, היינו שלמשך זמן מפסיק בזה ומתנהג שלא כדבריהם, וכבר מבואר הן בתורת הנגלה והן בתורת החסידות, אודות יוקר הגוף של איש הישראלי, שמילא צריך להשתדל שהי'י בריא ושלם, שאז אפשר לעבוד השי"ת מבלי בלבולים, ואדרבה כפירוש הבעש"ט עזוב תעזוב עמו, מיטן גוף, והנהגה הפכית בזמננו זה, הנה לפעמים קרובות יותר, סיבתה רצון צו ווייזען קונצען או גם גרוע מזה, ובמילא כלך מדרך זו, ועכ"פ מכאן ולהבא הנה ישאל את הרופא שלו הוראות מפורטות בהנהגה דאכילה ושתי' וכו', ובודאי יש לסדר שלא תהי' כל סתירה להשו"ע ח"ו וינהג גן, ובמילא וועט זיך בא אייך גיך אוישטיין ויחזור לאיתנו.
