An act is irreversible. (This letter is addressed to a Rav. He was to marry a man who had been ill and to whom a name had then been added. The Rav wished, in the kesubah*, to inscribe in the first place this added name, although it was not in use.) It concerns, in this case, an act of marriage and not of divorce.

In a get*, one cannot cite a name that is not at all borne by a man — if no one calls him thus, not even when he goes up to the Torah, if he does not sign that way.

In all the examples that you cite and which are also disputed, one likewise mentions the name that is actually borne, and adds the formula "hamechuneh*" — "who is called" — that is, the true name first, then the added name.

This point will not be developed here.

...ואין אחר מעשה כו', כיון שהוא בכתובה ולא בגט, שבגט הרי א"א לכתוק רק אופן שאין אף א ' קורא אותו או עולה לתורה או חותם כן; ובכל הדוגמאות שהביא כת"ר (ושגם בהם מחלוקת) מזכירים גם אופן שנק' בפועל, וכותבים דמתקרי וכו'.
